In order for your application to be considered, the following MUST be provided:

1. A narrative up to 500 words describing the achievements of the person or team.
2. Photocopy of newspaper clippings / or any non-returnable documentation.
3. Where possible submit letters of endorsement.

** Please submit a clear black and white photo (approx. 5” X 7”) (if possible) of
nominee(s).

Nominator's information

The deadline for submission is May 31 of each year. Submissions received after May 31 will be considered for the following year.

All nominees who are eligible for induction but are not selected for this year’s Sports Hall of Fame, will be kept on file for future consideration.
