The Fee Assistance Program provides a $300 credit per person toward Community Services registered activities, memberships or 10-ticket passes.
All family members who live in the household must be listed on the application. If you do not wish to recieve funding for certain family member, please select opt out beside their name.

All supporting documents must be from the current tax year.
Ontario Works and Ontario Disability Support Program must be from the current month.
